CONSENT
(TO THE COLLECTION AND LIMITED USE AND DISCLOSURE
OF PERSONAL INFORMATION)

For the purposes of facilitating compliance with the provisions of any applicable federal and/or provincial legislation governing the collection and use of personal information (including, without limitation, the Personal Information Protection and Electronic Documents Act, Ch. 5, S.C. 2000, as amended,) the undersigned does hereby consent to the collection, use and disclosure of personal information concerning the undersigned that is reasonably necessary to enable TSI Group, and its agents and employees, to proceed with the sourcing and placing of the undersigned in a job in connection with either a position specifically applied for by the undersigned, or any other position which TSI Group, and its agents and employees, may reasonably believe the undersigned is qualified; provided, however, that TSI Group, and its agents and employees, will first disclose to the undersigned, either in writing or orally, the identity of any prospective employer prior to the disclosure of any personal information to that prospective employer.

The undersigned does hereby consent to the collection, among other things, of the following information: the undersigned's name, home address, business address, occupation, employment and educational history, and name of employer, e-mail address, telefax/telephone number, residency status, and any other and further personal information that may be required to properly and adequately source and place the undersigned in a job opportunity, or to properly and adequately permit TSI Group to fulfill its obligations to either its prospective client employer or the undersigned, or as required by law, in the course of it reasonably rendering its services. This consent shall also permit TSI Group to collect such personal information as may be reasonably required in order to perform reasonable reference checks regarding the employment and educational history of the applicant, and any other information reasonably and legally required by a prospective employer.

The undersigned does hereby consent to the use, and or disclosure of, any personal information collected pursuant to this consent by TSI Group, and its agents and employees, to such third parties as reasonably required to properly and adequately source and place the undersigned with a prospective employer and any matters ancillary thereto for the purpose of finding and placing the undersigned in a job opportunity, including, without limitation: any companies or legal entities associated or involved with TSI Group in relation to the purposes hereof; any prospective employer or agent thereof working for the prospective employer for purposes of recruiting employees; and, any other person to whom the undersigned further consents to such disclosure or disclosure is required by law. TSI Group may also disclose any personal information voluntarily supplied to it by the undersigned for purposes of rendering its services to a prospective employer in relation to the undersigned. TSI Group shall limit disclosure of any personal information collected pursuant to the terms of this consent to such parties as are reasonably required to properly and adequately fulfill its obligations to the undersigned(s) in finding and arranging employment with a prospective employer, or any ancillary matters.

The undersigned further instructs any third party to disclose to TSI Group, its agents or employees, any personal information to which consent is hereby granted for collection of personal information by TSI Group without further consent being required from the undersigned. The undersigned hereby acknowledges that he/she has read and understood this consent (and all of its terms), and has voluntarily executed and delivered this consent to TSI Group. This consent, or a true copy thereof, shall constitute proof of the authority to collect and disclose personal information in accordance with its terms.
